Continue methotrexate 20 mg weekly with folic acid 1 mg daily and adalimumab 40 mg subcutaneously every two weeks. Recheck CBC, AST, ALT, and creatinine in 8 weeks per methotrexate monitoring. Continue vitamin D. Counseled on holding methotrexate during any significant infection. Return in 3 months, sooner if she has a flare. Repeat DAS28 at next visit to confirm she stays in target.
